Our mission is to provide comprehensive care to our patients across multiple phases of the healthcare system. Our hospitalist program provides compassionate inpatient care to all patients, 24 hours a day, 7 days a week. We identify patients with multiple chronic conditions who are in need of further support, and our providers will follow these patients closely in the outpatient setting, within the first 30 days of discharge. We follow up with these patients in a post-acute care clinic, and we also follow these patients as they transition to a skilled nursing facility.

Our team is comprised of multiple providers, including physicians, allied health professionals, case managers, pharmacists, and social workers. We utilize a coordinated, multi-disciplinary approach to identify potential gaps in transitions of care and chronic disease management. We carefully create specific, individually tailored plans for each patient, and collaborate with the patient’s family and other healthcare providers to ensure the plans are thorough but also easy to understand.

We emphasize robust collaboration with all members of the healthcare community and all stages of a patient’s care path. We utilize the full extent of healthcare technology to facilitate communication among a patient’s primary care physician, urgent care, emergency department, and specialists. We strive to ensure that all aspects of a patient’s care plan are connected, and transition seamlessly among each phase.

  • Dr. Kenneth Sim is the Executive Chairman of ApolloMed. He has been the Chairman of Network Medical Management (NMM), one of the company’s subsidiaries, since 2013 and has been a member of NMM’s Board of Directors since 2006. Dr. Sim also serves as the Chairman of the Board of APC. Dr. Sim is a Fellow of the American College of Surgeons and was awarded the Independent Physician Leadership Award in 2014 by the Los Angeles County Medical Association. Dr. Sim is also a member of the Governing Board of Directors at Alhambra Hospital Medical Center and a Board Member on the National Council of Asian Pacific Islander Physicians.
  • As an entrepreneur, Dr. Sim founded “Healthcare City” in the City of Industry, California, which helped streamline the healthcare process by providing outpatient health services at one location which included a surgical center, a senior wellness center and laboratory, radiology and urgent care services. He received his bachelor’s degree from the University of California, Los Angeles and received his medical training from the Loma Linda University School of Medicine and the Autonomous University of Guadalajara, Guadalajara, Mexico

  • Dr. Benson Kwee is the Medical Director of ApolloMed Hospitalist, a subsidiary of ApolloMed. Dr. Kwee has been a practicing hospitalist for ApolloMed Hospitalist since September 2017, and he has served as the Medical Director for ApolloMed Hospitalist since September 2021. Dr. Kwee is a member of the Medical Executive Committee at Alhambra Hospital Medical Center, Garfield Medical Center, and San Gabriel Valley Medical Center. Dr. Kwee received his medical degree from Lake Erie College of Osteopathic Medicine and completed his family medicine residency training at Summa Health System, Akron City Hospital.

  • Dr. Thomas Lam is the Co-Chief Executive Officer and President of ApolloMed. Dr. Thomas Lam has been a member of ApolloMed’s Board of Directors since January 2016. Dr. Lam has served as Chief Executive Officer of NMM since January 2006 and has been a member of NMM’s board of directors since 2005. From January 2006 to September 2014, Dr. Lam was the Chairman and CEO of APC. Since October 2014, he has served as the Chief Executive Officer and Chief Financial Officer of APC.
  • Dr. Lam was the recipient of the Corporate Citizens of the Year Award from the Board of Directors of East Los Angeles College Foundation in April 2014. In February 2015, the YMCA Board of Directors of West San Gabriel Valley honored Dr. Lam as the recipient of the Heart of the Community Award. Dr. Lam received his medical training from New York Medical College and gastroenterology training from Georgetown University. Dr. Lam serves as the nominee of NMM.

  • Dr. Patrick Pham is the Co-Medical Director of ApolloMed Hospitalist. Dr. Pham has been a practicing hospitalist for ApolloMed Hospitalist since July 2019, served as associate medical director for ApolloMed Hospitalist from September 2021 to June 2022, and assumed the role of Co-Medical Director for ApolloMed Hospitalist since July 2022. Dr. Pham is also the Medical Director for Sunny Village Care Center, a CMS 5-star rated Skilled Nursing Facility in Alhambra, CA. Dr. Pham is also a member of the Medical Executive Committee at Garfield Medical Center. Dr. Pham received his medical degree from St. George’s University School of Medicine and completed his internal medicine residency training at University of California, Riverside.

Seeking interested physicians, nurse practitioners, and physician assistants.
Position Details:
– Average census: 15-20 patients
– Full intensivist support. No codes, no procedures.
– Multispecialty support, including cardiothoracic surgery, hepatobiliary surgery, and neuro-interventional radiology
– 7 on 7 off schedule with option to do extra shifts
– No overnight calls
– Malpractice coverage with tail is provided.
– Cantonese, Mandarin, and/or Spanish speaking are preferred, but not required
Requirements:
– Unrestricted California State Medical License
– Candidates without a California State Medical License will be considered, however, the licensing process takes ~6 months
– Current California DEA certificate required to start

– 7 on 7 off schedule with option to do extra shifts.
– Nocturnist shift is 8 PM to 8 AM.
– Nocturnists admit 3-6 patients per shift.
– Malpractice coverage with tail is provided.
– Cantonese, Mandarin, and/or Spanish speaking are preferred, but not required
Requirements:
– Unrestricted California State Medical License
– Candidates without a California State Medical License will be considered, however, the licensing process takes ~6 months

Apollo360 Hospital Medicine is the flagship service of Apollo360. Our hospitalist physicians provide continuous acute care coverage to hospitalized patients year-round. It focuses on clinical care, education and quality of care, and patient safety in the hospital environment.

We work closely with case managers, social workers, and other members of the hospital care coordination team to ensure high-quality care and patient safety and provide a smooth transition of care when patients are discharged from the hospital.

We have a strong focus on evidence-based, appropriate utilization of diagnostic tests and medical treatment and formulate a care plan that is specifically tailored to each individual patient’s needs.

Apollo360 Post-Acute Care Clinic focuses on close follow-up of patients recently discharged from the hospital. Our program identifies patients with multiple chronic conditions and those with very complex hospitalizations.

Our post-acute care team proactively reaches out to these patients and provides a physician follow-up office visit within one week of discharge. Our team of physicians, allied health professionals, RNs, case managers, and social workers will work together to ensure a smooth care transition. They will also review their hospital stay and post-discharge instructions to increase patient compliance. We also provide medication reconciliation. We ensure that patients and their caregivers are confident and comfortable with the care plan.

By offering the entire post-acute continuum of care, we help ease the transition between healthcare providers by better aligning our physicians, hospitals, and post-acute care settings according to the patient’s needs. This collaboration is designed to streamline the transfer of care and reduce hospital readmissions.

Post-acute SNF care plays a critical role in health care by providing services that help get patients back to good health. The main focus is to maximize the patient’s functional abilities as well as confidence in caring for themselves as independently as possible. Apollo360 works closely with skilled nursing facilities in the San Gabriel Valley area to ensure a smooth transition of care after a patient is discharged from the hospital. Our physicians, nurse practitioners, and physician assistants regularly assess patients for skilled nursing needs while the patients are still hospitalized. We work closely with SNFs to ensure that all the medications and medical equipment are available for the patients when they arrive at the SNF. Our care team continues to see these patients at the SNF on a regular basis in order to ensure patients are receiving the treatments they need to improve their functional abilities with the ideal goal of transitioning these patients to a safe environment at home.
